新型コロナワクチン職域接種 予約受付(3回目接種)

今回の対象者はすでに新型コロナワクチンを2回接種済みの方で、2回目接種日から5か月経過した日以降の日程を予約してください。
予約が完了しても接種日に接種券(付き予診票)を持参できない場合は接種できません。(1回目、2回目のときのように接種券なしでの接種はできません)なお、接種券(付き予診票)の発送時期は、住民票のある自治体ホームページ等でご確認ください。
(Please make the reservation the date after over 5 months past from 2nd vaccination. If you can’t bring the vaccination coupon with pre-examination form (Yoshinhyo), you cannot take the shots even if the reservation has completed (not the same as 1st and 2nd). Please check the local government website where there is your resident register when the coupon will be sent.)
