(Entry for Medical Check-up Reservation starts from 12:00pm, 25th Mar.)

2.お申込み方法(How to apply)

個人情報の取り扱いに同意の上、お申し込みください。
(Please apply after agreeing to the handling of personal information.)

男性の方(For male)

女性の方(For female)

3.受付内容の確認・キャンセルについて(Confirmation of reception contents・Cancel)

　お申し込みの際に登録したメールアドレスとパスワードで、ログインしてください。
　(Please login with the e-mail address and password you registered on the reservation form.)

4.お問い合わせ(Father)
